What’s Required to Begin

We’re committed to doing this right—not just for your needs, but to ensure ESA documentation remains clinically appropriate and legally valid.

You’ll need to:
  • Complete 2 therapy sessions with an ACC clinician

  • Pay an out-of-pocket fee per document requested

  • Be an active client with insurance or private pay

  • Fill out our biopsychosocial intake form for ESA support

What This Documentation Covers

We provide ESA letters for use in:

  • Housing accommodations

  • HOAs or landlord requests

  • College dormitory or campus housing exceptions

Our documentation is:

  • Based on a thorough clinical evaluation

  • Tailored to your mental health needs

  • Finalized and delivered as a PDF directly to you (and, if applicable, your residential facility)

In some cases, your clinician may recommend an additional session if more information is needed to appropriately assess your request.
